Retirement Information

Dear Patients,

Please be advised that I will be retiring on January 31st, 2023. It has been an honor and great privilege to provide neurology care for Alaskans over the last 20 years.

If you haven’t received a letter from me with an ‘Authorization to use and/or Disclose Health Information’ form enclosed, please contact my office to request one.

Once completed and returned a copy of this form will be maintained in your medical so they can be sent to your new neurology provider.

A list of possible Neurologist Options is provided below.

We will provide as seamless as possible transition for your continued neurology care and needs. I will retain your confidential medical records for seven years as is required by law.

Thank you for allowing me to provide 20 years of neurology service for you and our community.
